Who Is On Your Care Team?
Hospice Physician: Oversees your medical care plan, manages pain and symptoms, and works closely with the rest of the care team to ensure your needs are met.
Registered Nurse (RN): Provides regular assessments, administers medications, and offers education and support to both patients and families.
Medical Social Worker: Offers emotional support, counseling, help with planning, and assistance accessing resources like financial aid or community support services.
Home Health Aide: Assists with daily personal care activities such as bathing, dressing, grooming, and light housekeeping to maintain comfort and dignity.
Chaplain / Spiritual Adviser: Offers spiritual and emotional guidance, respecting your beliefs and providing comfort regardless of faith or religious background.
What to Expect During Visits
Scheduled team visits tailored to your needs, with each member (nurse, aide, social worker, chaplain, physician) visiting on a routine basis to provide focused care and support.
Flexible visit frequency that adjusts over time based on your changing health, comfort levels, and family needs.
24/7 on-call access to a hospice nurse for urgent medical concerns, pain management issues, or emotional support at any hour, day or night.
Ongoing communication and updates shared between our care team, your primary physician, and your loved ones to keep everyone informed and involved.
